Heaney Retractor

The Heaney is an angled handheld vaginal retractor used to displace a vaginal wall while preserving a working view. Its blade and handle are oriented so the assistant can apply directed traction without placing a hand directly in front of the operative target. It is distinct from the Heaney clamp and Heaney needle holder.

Design and size selection​

The name is applied to more than one configuration. Aesculap lists EL865R, a Heaney vaginal speculum with 83 mm working length, 29 mm blade width and 170 mm overall length. Teleflex also supplies longer Heaney-Simon retractors, including a catalog model with a 120 mm deep blade and 280 mm overall length.[1][2]

Check blade shape and reach on the actual tray. It is inaccurate to assume that every Heaney is shorter than every Breisky. The blade is held manually; the instrument does not maintain exposure by itself.

Practical applications​

  • Anterior, posterior or lateral vaginal-wall retraction during vaginal reconstruction.
  • Directed exposure during vaginal hysterectomy, cuff work or colporrhaphy.
  • Retraction of a mobilized vaginal wall or adjacent tissue during fistula repair, according to the dissection plane.
  • Supplementing a weighted speculum or another blade when one direction of exposure is insufficient.

The blade should support an identified tissue surface.

Handling​

  1. Select enough blade depth to reach the target without obscuring the surrounding anatomy.
  2. Place the tip under direct vision and maintain broad tissue contact.
  3. Coordinate the direction and amount of traction with the surgeon; avoid sudden handle movement during needle passage or dissection.
  4. Reposition instead of levering harder when tissue slides over the blade or the target disappears.
  5. Reassess pressure during prolonged exposure and release traction before withdrawal or exchange.

A narrow blade can concentrate pressure; a broad one can obscure a confined field. If the handle or blade repeatedly crowds the target, a different length, Breisky, Sims, or appropriately configured self-retaining system may be more useful.

Bladder and rectum retraction are common roles in fistula work, for example displacing the bladder during vesicovaginal fistula repair or the rectum during rectovaginal fistula repair.
