Contributions

Grimes studies the pathophysiology of urethral stricture disease and lichen sclerosus, including stricture histology and collagen structure, and reports ureteral reconstruction outcomes in radiated and non-radiated patients.789

  • Histopathology of anterior urethral strictures (2019). Led a blinded pathology review of 100 urethroplasty specimens: 21% had two or more lichen sclerosus features and 44% showed chronic inflammation, which was largely absent from isolated bulbomembranous strictures (9%); the 11% failure rate did not differ with inflammation.7 Cited on WARWIKI: Male Urethral Stricture.
  • Ureteral reconstruction after radiation (2022). In 47 consecutive single-surgeon ureteral reconstructions, radiated patients more often needed complex repairs (ileal ureter or autotransplantation; 35% vs. 7%), and success was 94% in radiated and 100% in non-radiated patients at a median follow-up of 30 months.8
  • Collagen in lichen sclerosus (2023). As senior author, showed greater collagen content and less homogeneous, more disorganized collagen structure in urethral, vulvar and foreskin tissue affected by lichen sclerosus than in unaffected tissue.9
